29 CFR 1928.57(a)(6): At the time of initial assignment, the employer did not instruct every employee in the safe operation and servicing of all covered equipment with which he is or will be involved, including at least the following safe operating practices: a) On or about October 3, 2019 and at times prior thereto; agricultural machine operators were exposed to crushing injuries during cleaning and clearing of a jam while in the field using a Double L Windrower 6540, SN 251174063182; the employer did not ensure that the power source was disconnected and wait for machine movement to stop prior to agricultural machine operator clearing a jam in the conveyor belt. The employer did not ENSURE THE CONTROL OF HYDRAULIC DRIVEN ENERGY before entering the machine's conveyor system to clear the jam created by a metal bar used to clear debris, roots and rocks. 29 CFR 1928.57(a)(11): Employer did not ensure moving machinery parts did not present a hazard during servicing or maintenance, the engine WAS NOT stopped, the power source disconnected, and all machine movement stopped before servicing or maintenance is performed. a) Double L Windrower 6540, SN 251174063182, potato harvesting field in Burley, Idaho, on or about October 3, 2019 and at times prior thereto: Employer did not protect agricultural machine operators from crushing injuries during maintenance and servicing of the machine when employer attempted to dislodge a metal tool from the conveyor belt while the power source was still connected. Residential energy caused the belt to shift. Note* Abatement certification IS required for this item. |
