29 CFR 1910.147(f)(2)(i): When outside servicing personnel were engaged in activities in a facility, the onsite employer and the outside employer did not inform each other of their respective lockout or tagout procedures: a) Grain Handling facility: On March 28, 2023 and times prior, the employer Liberty Basin, LLC, had not developed or implemented an energy control program of their own which would include specific written procedures for identifying and controlling the energy sources for the equipment that had been installed or re-located by the sub-contractor(s). Items identified included but were not limited to, the ground corn drag conveyor leaking an excessive amount of combustible dust from a damaged seal for an undetermined amount of time. b) Grain Handling Facility: On March 28, 2023, and at times prior the main dust collection system was not functioning properly for approximately four (4) months. This resulted in the baghouse socks, auger and associated piping to be continually clogged, "packed off" until the problem was corrected, and the baghouse socks were replaced.
