These components of the fall protection system were not designed for this application. b) Northeast corner of sixth floor: On February 24, 2016 employees performing framing activities wore improperly adjusted and improperly installed rope grab fall protection systems. The rope grab mechanism was adjusted to the very end of the rope lanyard. The snap hook on the 3 foot lanyard was then tied back to the rope lanyard just above the rope grab mechanism.
