29 CFR 1926.451(c)(2)(v): The employer used forklifts to support scaffold platforms without ensuring that the entire platform was attached to the fork and that the forklift was not moved horizontally while the platform was occupied: a) On June 26, 2023; employees accessing a second story roof and working from an elevated platform supported by a rough terrain forklift while installing sheathing during framing activities, were exposed to falls of approximately 17 feet. More than half the length of the platform extended forward, approximately 10 feet past the end of the tines. |

The platforms left side, measuring approximately 15 feet long, and end did not have guardrails. Employees were not wearing personal fall protection. |
