The total weight of the work basket, the boom angle, boom length and position of the outriggers combined, exceeded the rated lift capacity load as specified for this machine. |

The platform was not approved by the manufacturer and the employer did not evaluate what effect the additional weight and overall length of the work basket would have on the rated lift capacity, the stability and safe operation of the machine. |

The telehandler operator and the worker in the basket had not received scaffold related training and did not recognize the stability hazards when using this machine with this basket attached and take the precautions needed to minimize these hazards. |
