Citation text line OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck-by and crushing hazards while performing material handling activities in the warehouse: a) Warehouse: On November 20, 2012 and at times prior thereto employees were exposed to a crushing and struck-by hazard from improperly installed and maintained components of the industrial storage rack system. Beams were not properly installed and maintained in that safety bolts and other safety mechanisms were either not in place or not positively engaged with their supporting upright columns.
