On or about February 11, 2013, and at times prior thereto, the discharge from the two pressure safety relief valves of the Ammonia High Pressure Receiver (HPR) was not piped to a safe location. The discharge from the valves was immediately above the HPR. b.) On or about February 11, 2013, and at times prior thereto, the door from the Anhydrous Ammonia Machinery Room to the manufacturing facility, was not an approved self-closing, tight-fitting fire door which opened outward.
