Citation text line 29 CFR 1926.454(b)(2): The employer did not have each employee who was involved with erecting, disassembling, moving, operating, repairing, maintaining, or inspecting a scaffold trained by a competent person to recognize the correct procedures for the work and type of scaffold in question: a) Jobsite: On February 28, 2017 and at times prior thereto employees working from a tubular welded scaffold system were exposed to a potential failure of the scaffold. Employees and the competent person did not recognize or acknowledge the hazard of having multiple base plates without mud sills on soft, muddy ground. Actions were not taken to correct or minimize the hazard. b) Jobsite: On February 28, 2017 and at times prior thereto employees working from a tubular welded scaffold system were exposed to a potential failure of the scaffold. The varying ground conditions, along with the condition of the scaffold and the load on the scaffold from workers, tools and materials, allowed the scaffold to begin to shift, sink and lean. The competent person on site did not recognize or acknowledge these conditions and take actions to correct or minimize the hazard. c) Jobsite: On February 28, 2017 and at times prior thereto employees working from a tubular welded scaffold system were exposed to a potential fall hazard and failure of the scaffold. Employees climbed the scaffold frame for access to the working level. Employees and the competent person did not recognize or acknowledge the hazard and take actions to correct or minimize the hazard. Note) Certification of corrective action IS required for this item.
