29 CFR 1926.251(a)(6): Each day before being used, and during use, where service conditions warrant, the sling and all fastenings and attachments were not inspected for damage or defects by a competent person, and damaged or defective slings were not immediately removed from service: a) Jobsite: On or about October 31, 2017 and at times prior thereto employees performed well drilling activities using an Ingersoll Rand T3 well drill. A competent person had not completed an inspection prior to using the sling (wire cable) and associated hardware. The correct number of U-bolt clips were not in place on a 5/16 inch wire cable used to hoist and hold drill stems. U-bolt clips were installed backwards and not properly spaced on a 1/2 inch wire cable also used to hoist and support drill stems and/or casings. b) Jobsite: On or about October 31, 2017 and at times prior thereto employees performed well drilling activities using an Ingersoll Rand T3 well drill. A competent person had not completed an inspection prior to using the sling (wire cable) and associated hardware. The 5/16 wire cable that hoisted and supported the drill stem was looped. The defective wire cable had not been identified and immediately corrected or removed from service. c) Jobsite: On or about October 31, 2017 and at times prior thereto employees performed well drilling activities using an Ingersoll Rand T3 well drill. A competent person had not completed an inspection prior to using the sling (chains) and associated hardware. The three chains used around the casing were not positively secured to the casing clamp to prevent the chains from becoming slack or dislodged, before and through-out the well drilling activities. Note) Certification and documentation of corrective action are required. |
