29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: (a) On June 13, 2024, employees removing shingles from a single-family building were using an extension ladder to access and exit the roof. The ladder was propped against the eave with an approximate 2.5ft extension above the access point. This condition exposed employees to fall hazards of 9.5 feet.
