Citation text line 29 CFR 1926.451(a)(6): Scaffolds were not designed by a qualified person and were not constructed and loaded in accordance with that design: a) Jobsite: On July 27, 2017 and at times prior thereto the employer used a sheet of oriented strand board (OSB) attached to a rough terrain forklift as a scaffold. The employees used the scaffold for access up to the 3rd floor work area. The scaffold platform was not designed and constructed by a qualified person in that a guardrail system or other fall protection system was not in place. The employees were exposed falls hazards ranging from 14 feet to the lower level and 24 feet to the ground below. b) Jobsite: On July 27, 2017 and a times prior thereto employees performing faming activities used a scaffold platform that was supported by a rough terrain forklift for access to the 3rd floor work area. The scaffold platform was not designed and used by a qualified person in that the forklift manufacturer, Genie, warned of a personal injury hazard, prohibiting people from riding on the machine, other than at the operator's station, and no passengers are permitted to ride on machine or any attachment. The employees were exposed falls hazards ranging from 14 feet to the lower level and 24 feet to the ground below. Note) Certification of corrective action is required for this item.
