(a) Front (east facing) roof: On or about January 24, 2014 and at times prior thereto, a roof anchor mounted to the peak and being utilized by two workers, doing siding activities between 10 and 15 feet from the ground, was not designed to accomodate more than one worker in the event of a fall. (b) Side (north facing) roof: On or about January 24, 2014 and at times prior thereto, a roof anchor mounted to the side of the facia was being utilized by two workers doing siding activities between 10 and 15 feet from the ground and was not designed to accomodate more than one worker in the event of a fall. (c) Side (north facing) roof: On or about January 24, 2014 and at times prior thereto, a roof anchor mounted to the side of the facia was not designed to withstand a force of 5000 pounds when mounted at this angle.
