29 CFR 1926.501(b)(1): Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems: a) Conveyor dryer: On or about December 30, 2016 and at times prior thereto employees performed electrical installation activities on top of an unprotected walking/working surface without a fall protection system in place. Employees were exposed to a 12 foot fall to the concrete below. Note ) Certification of corrective action is required for this item. |

29 CFR 1926.503(a)(2)(iii): The employer did not assure that each employee had been trained, as necessary, by a competent person qualified in the use and operation of guardrail systems, personal fall arrest systems, safety net systems, warning line systems, safety monitoring systems, controlled access zones, and other protection to be used: a) Conveyor dryer: On or about December 30, 2016 and at times prior thereto employees did not recognize or acknowledge the hazard of working on an unprotected walking/working surface greater than six feet to the lower level, without a fall protection system in place. Employees did not take action to correct or minimize the hazard. b) Conveyor dryer: On or about December 30, 2016 and at times prior thereto employees used a personal fall arrest system for protection from a 12 foot fall hazard. The employer and employees did not recognize or acknowledge the difference between a fall arrest and fall restraint system and the hazards of using the wrong type of a fall protection system. Note) Certification of corrective action is required for this item. |
