In energized installation(s), each outlet box did not have a cover, faceplate, or fixture canopy: a) Jobsite: On September 21, 2011 and times prior thereto the creating employer, George's Electirc, Inc., did not install a proper cover on the circuit breaker panelboard in the garage, exposing employees to the energized circuits inside. b) Jobsite: On September 21, 2011 and times prior thereto the creating employer, George's Electirc, Inc., did not install face plates on two outlets in the garage, exposing employees to the energized circuits.
