(a) On or about June 11, 2012, and at times prior thereto, the exposing employer did not provide employees with adequate fall protection while working on the top plate, on the front side of the residence, placing blocks between trusses at heights of approximatley 17 feet. (b) On or about June 11, 2012, and at times prior thereto, the exposing employer did not provide employees with adequate fall protection while working on the trusses, near the middle of the residence, at heights of approximatley 17 feet above the concrete foundation.
