(a) 1812 N. Lakewood Dr.: On or about May 30, 2018 and at times prior, at approximately 30 ft. employees were working on a scaffold with sections that did not have a top rail. The 30 inch cross member met the height of a mid-rail, but it did not have a top rail. (b) 1812 N. Lakewood Dr.: On or about May 30, 2018 and at times prior, at approximately 30 ft. employees were working on a scaffold with sections that did not have a mid rail.
