A single Honeywell Gas Alert MAX XTII monitor was used for continuous air monitoring. It was located approximately 15 feet above the area where the authorized entrants were working. Credence Clearwater Industrial employees entered and performed sludge removal activities in a waste water treatment digester tank under a confined space entry permit generated and maintained by the controlling employer. Citation text line 29 CFR 1926.1207(a): The employer did not provide training to each employee whose work is regulated by this standard, at no cost to the employee, to ensure that the employee possessed the understanding, knowledge, and skills necessary for the safe performance of the duties assigned under this standard. This training must result in an understanding of the hazards in the permit space and the methods used to isolate, control or in other ways protect employees from these hazards, and for those employees not authorized to perform entry rescues, in the dangers of attempting such rescue a) On March 9, 2021 and at times prior thereto the employer had not developed or implemented their own employee training program for confined space, permit required confined space, and confined space entry. The employer did not provide training for their employees that could then be supplemented, as needed, by a host or controlling employer to identify specific hazards and control methods unique to the jobsite and space being entered. Employees performed sludge removal activities in a waste water treatment digester tank. Note) Certification of corrective action is required for this item.
