An inspection was not conducted by the competent person prior to the start of work and as needed throughout the shift. a) On or about May 10, 2017 or prior thereto: Potential cave-in hazards were present on a 9 foot deep trench such as vertical walls, fissuring and spoil piles on the edge of the trench, and an inspection was not conducted for these conditions by a competent person in trenching and excavations. In addition, soil classification was not conducted, work was being conducted outside of the trench shielding method, and the shielding in use was not firmly braced up against the side walls of the trench.
