29 CFR 1926.501(b)(4)(ii): Each employee on a walking/working surface was not protected from tripping in or stepping into or through holes (including skylights) by covers: (a) Barrel roof at 710 N Orchard, Boise: On 05/05/11 and at times prior thereto, irregularly shaped holes, approximately 14.5 inches in diameter, were cut through the surface of the roof deck and left uncovered; two in the Northwest corner, two near the roof ridge. (b) Barrel roof at 710 N Orchard, Boise: On 05/05/11 and at times prior thereto, two vent covers were removed from 12 vent stacks which exposed holes in the roof. Note: Abatement certification IS required for this item. |
