The excavation measured between 8 1/2 and 9-feet in depth, 27 - 37 inches wide, and approximately 45 feet long. Employees did not recognize the hazards of working outside of the cave-in protection that was provided and did not recognize that the shoring system was not installed in accordance with the tabulated data. |
