29 CFR 1926.501(b)(10): Except as otherwise provided in paragraph (b) of this section, each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system. a) On or about October 18, 2017 or prior thereto: Employees were engaged in roof demolition activity on a low-slope roof having dimensions of 50 feet by 120 feet. The north side was marked with a warning line 6 feet from the edge. The employer had utilized a safety monitoring system on the roof. However, temporary employees were not trained in the safety monitoring system in use in the workplace. b) On or about October 18, 2017 or prior thereto: Employees were working adjacent to the east parapet while engaged in roof demolition activity on a roof having dimensions of 50 feet by 120 feet. The parapet on the east side of the building was up to 24 inches high. Employees were not wearing fall protection. Potential fall hazards were over 25 feet to an adjacent building. There was not a warning line system on the east side of the building. Temporary employees were not trained in the safety monitoring system in use in the workplace. c) On or about October 18, 2017 or prior thereto: Employees were working within 6 feet of the south roof edge while engaged in roof demolition activity on a roof having dimensions of 50 feet by 120 feet. The parapet on the south side of the building was approximately 9 inches high. Employees were not wearing fall protection. Potential fall hazards were over 40 feet to ground level. There was not a warning line system on the south side of the building. Temporary employees were not trained in the safety monitoring system in use in the workplace. d) On or about October 18, 2017 or prior thereto: Employees were working within 4 feet of the west roof parapet while engaged in roof demolition activity on a roof having dimensions of 50 feet by 120 feet. The parapet on the west side of the building was approximately 18 inches high. Employees were not wearing fall protection. Potential fall hazards into an interior skylight pit were over 25 feet. There was not a warning line system on the west side of the building. Temporary employees were not trained in the safety monitoring system in use in the workplace.
