Wright Brothers employees and a sub-contractor worked at a wall opening on the west end of the second floor, unloading construction materials from a rough terrain forklift into the building. No fall protection system was being used. This exposed them to a 12 foot fall hazard. b) Jobsite: On May 2, 2018 and a times prior thereto the controlling employer Wright Brothers, The Building Company did not identify hazards through an effective frequent and regular inspection program. Hazards included, but may not be limited to, sub-contractors not wearing head protection when needed. c) Jobsite: On May 2, 2018 and a times prior thereto the controlling employer Wright Brothers, The Building Company did not identify hazards through an effective frequent and regular inspection program.
