Hazards included employees not using fall protection, employees using unserviceable fall protection components and employees using non-compatible fall protection components. b) On March 12, 2024, and times prior the employer did not effectively maintain the site inspection system to identify and correct hazards to which their employees were exposed. The employee did not recognize the hazards of using unserviceable personal fall arrest components or the actions needed to correct the hazard. b) On March 12, 2024, two employees were attached to the same anchor point. The employees did not recognize the hazards of not properly using personal fall arrest or the actions needed to correct the hazard. c) On March 12, 2024, and times prior employees used fall protection components that were not compatible, in that rebar hooks were attached to snap-hooks. The employees did not recognize the hazard of not properly using personal fall arrest or the actions needed to correct the hazard. d) On March 12, 2024, and times prior an employee used the snap-hook attached back onto the lanyard as part of the anchor point.
