The guarding device was not in conformity with any appropriate standards therefor, or, in the absence of applicable specific standards, was not designed and constructed as to prevent the operator from having any part of his body in the danger zone during the operating cycle. a) Manufacturing Area, on or about July 21, 2015 or prior thereto: The Powermatic Table Saw, Model #66 Serial #8966502, was missing an overhead guard to protect the operator during the sawing of plastic. b) Manufacturing Area, on or about July 21, 2015 or prior thereto: The Mazak Quick Turn 15 N horizontal turning machine had its disconnect switch by-passed, exposing operators rotating parts with the front door open.
