29 CFR 1926.403(b)(1): The employer did not ensure that electrical equipment is free from recognized hazards that are likely to cause death or serious physical harm to employees. a) On or about February 28, 2017, or prior thereto, A-Core of Boise, Inc. Office and Equipment Warehouse: A 10 gauge extension cord, or pigtail, was not tested for correct attachment of the grounding conductor. The L2 Hot and Grounding Conductor were reversed during installation. The pigtail was designed by A-Core of Boise, Inc. to be used with a Husqvarna Power Pack PP65, and a Husqvarna K6500 HF High Frequency Saw. b) On or about March 8, 2017, or prior thereto, First Healthy Choice Site: The grounding conductor and the L2 hot line on a 10 gauge extension cord, or pigtail, were reversed during concrete cutting work. c) On or about March 28, 2017 or prior thereto, Micron Technology Building #36: The grounding conductor and the L2 hot line on a 480 Volt 10 gauge extension cord, or pigtail, were reversed. This resulted in energizing the casing of the Husqvarna Power Pack PP 65 during concrete cutting work. NOTE: This item was corrected during the inspection. |
