Citation text line OSH ACT of 1970 Section (5)(a)(1):The employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to electrical hazards such as arc flash and electrical shock. a) On July 10, 2023, and at times prior, employees performing shutdown maintenance, testing, and repair of electrical equipment were exposed to potential electrical hazards while working on electrical systems of up to 480 Volts without an adequate shock hazard assessment or arc flash risk assessment as described in NFPA 70E (Standard for Electrical Safety in the Workplace Article 130.4 and 130.5 2021 Edition) Workplace assessments had not been conducted to determine whether the potential hazards of the work to be performed warranted the use of any, or all such personal protective equipment, specialized tools, barriers, shields or insulating materials were needed to protect employees.
