The toprail was 31 inches above the platform and did not comply with requirements for guardrail systems in 29 CFR 1910.29(b)(1) and (b)(2). The platform was located above an onion hopper with a conveyor system. b) On or about October 19, 2018, or prior thereto: The Onion Bin Tipping Machine had a 12 inch wide worker platform with an inadequate guardrail system. The platform was not guarded by a mid-rail, and did not comply with requirements for guardrail systems in 29 CFR 1910.29(b)(1) and (b)(2). The platform was located above an onion hopper with a conveyor system. c) On or about October 19, 2018, or prior thereto: The Onion Bin Tipping Machine had a 12 inch wide worker platform with an inadequate guardrail system. The top rail was 31 inches above the platform and was offset on an angle to the walking/working surface. The top rail was not perpendicular to the platform. 29 CFR 1910.212(a)(1): One or more methods of machine guarding was not be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. a) On or about October 19, 2018, or prior thereto: The metal frame work of the Onion Bin Tipping machine was not adequately guarded at ground level. Workers were not protected from nip points or shearing actions created by the structural members of the frame when wood onion bins were lowered by hydraulic action. Workers access this area to sweep onion residue. b) On or about October 19, 2018, or prior thereto: The return conveyor entering the Onion Bin Tipping Room had an ingoing nip point on the underside of this conveyor. Workers were exposed to an ingoing nip point on a pleated conveyor that was measured at 79 inches above ground level. The underside of this conveyor was not protected or guarded. Workers access this area to sweep onion residue. c) On or about October 19, 2018, or prior thereto: The conveyor fed by the return conveyor had an underside, or return feed, that was was not protected or guarded. Workers were exposed to a pleated conveyor that was measured at 56 inches above ground level above an aisle way. The underside of this conveyor was not protected or guarded. Workers access this area to sweep onion residue. NOTE: Abatement certification and documentation are required for this item. |
