Employees working within the trench box were exposed to a collapsing hazard from the 4.5 feet of unprotected trench wall. b) Jobsite: On June 6, 2019 and at times prior thereto employees worked in a 12.5 foot deep trench where only a single, 8 foot high trench box was used. Employees working within the trench box were exposed to a collapsing hazard from the spoil pile that was placed at the edge of the trench/excavation. c) Jobsite: On June 6, 2019 and at times prior thereto employees worked in areas of a 12.5 to 15 foot deep excavation where cave-in protection was not in place.
