(a) Feed Mill, on July 17, 2012, and at times prior thereto, employees were exposed to combustible dust hazards in a feed mixing area where there were 1/2 inch to over 3 inch accumulations of Class II combustible dusts. No documented housekeeping plan or program was in place to minimize dust accumulations. (b) Feed Mill, on July 17, 2012, and at times prior thereto, employees were exposed to combustible dust hazards in a feed mixing area where 1/2 inch to over 3 inch accumulations of Class II combustible dusts were routinely cleaned with compressed air. No safety procedures were in place to eliminate sources of ignition before using the compressed air for blowing down the dust accumulations. (c) Feed Mill, on July 17, 2012, and at times prior thereto, employees exposed to combustible dust hazards in a feed mixing area where there were 1/2 inch to over 3 inch accumulations of Class II combustible dusts. No warning signs of potential dangers were posted at the entrances to the building . (d) Feed Mill, on July 17, 2012, and at times prior thereto, employees were exposed to a feed mixing area where there were 1/2 inch to over 3 inch accumulations of Class II combustible dusts. The pendant light and the trouble light near the Big Mixer were not approved for a hazardous location. |
