Notes INITIAL REVIEW ORDER BY SCREENING JUDGE Plaintiff's Motion for Appointment of Counsel (Dkt. 4) is DENIED. Plaintiff's Motion to Proceed on Partial Payment of Court Fees (Dkt. 5) is DENIED for the reasons stated in the Court's ordistered Participants at the addresses listed on the Notice of Electronic Filing (NEF) by (jp)er denying Plaintiff's in forma pauperis application. Plaintiff's motion for preliminary injunctive relief (Dkt. 6) is DENIED. Plaintiff's Motion for Extension of Time to Acquire Funds to Proceed (Dkt. 11) is GRANTED. Plaintiff may proceed on her Eighth Amendment failure-to-protect and corresponding negligence claims against Defendants Blair, Taylor, Sanders, and Martin. She may also proceed on her medical treatment claims, as well as her corresponding state law claims for negligence or medical malpractice, against Defendants Alviso, Eldridge, Siegert, Campbell, and Atencio. All other claims and all other Defendants are DISMISSED. Jeremy Clark, Haughn, Management Treatment Committee, Jennifer Tyvand, Valley, Watson, Howard Yordy, Administrative Review Committee and Marc Aiello terminated. Plaintiff is responsible for serving a summons and a copy of the Complaint upon Defendants Blair, Taylor, Sanders, Martin, Alviso, Eldridge, Siegert, Campbell, and Atencio, either by formal service of process or by obtaining signed waivers of service of summons from each Defendant or theircounsel. Plaintiff must serve these Defendants within 90 days after entryof this Order.
