29 CFR 1910.269(e)(4): Before any entrance cover to an enclosed space is removed, the employer did not determine whether it was safe to do so by checking for the presence of any atmospheric pressure or temperature differences and by evaluating whether there might be a hazardous atmosphere in the space.
